  • Please Note: KJ Therapeutics ONLY services Women. Sorry gentleman, but the ladies need some attention.

  • Booking can be completed by contacting the clinic via phone, email, or by sending in an appointment request via the websites contact link.

  • Upon booking you will receive a text message and/or email confirmation (depending on how you would like to be notified) for your appointment. It will come from a different number but will contain the clinics name provided in your notifications powered by PocketSuite.

  • A General Health Intake form and Contract for Consent to Treat will be sent to you via your chosen and consented form of contact.

  • When these forms send, the link may look like spam. The link will take you to a white page with the KJ Therapeutics up at the top, a line along the page with view document and sign at the bottom. View the document to read the contract in full or fill out your intake form. Exit the form and sign on the previous page.

    • If you would prefer fill out and sign documents as a paper form, no problem! I can bring printed copies to you for our initial visit.
